1. Challan for Rs: 1000/- Paid at SBI, Anneparthy Branch, Nalgonda.
2. Application Form (with extra one Photo).
3. Previous Certificate (Degree Convocation only).
4. Present Completed Consolidated Memo and Provisional (PG).
5. Student Aadhaar Card (Optional).
6. SSC Memo / Intermediate / Mother Aadhaar Card (Proof of Mother Name in case if it is not mentioned in previous study certificates).
Note: All the Xerox copies need to Attested by Gazetted Officer.
Minimum Time to issue the Certificate: ONE MONTH
